School & Sports Physicals

Physicals and immunizations are scheduled each summer. To schedule an appointment for the school year, please call the Livingston County Health Department at (815) 842-5959

Types of physicals available include:

• Preschool/Daycare/HeadStart
• Foster Care
• Kindergarten
• Sports Physicals for Grade School, High School, and College
• Freshman Physical for High School and College

By offering comprehensive health services to area children today, health problems will be prevented in the future.

Clinic services include: 

• Hemoglobin tests 
• Blood lead screenings
• Immunizations
• TB Waiver if needed

Charges will be billed to private insurance or Medicaid when available.

 Reasonable accommodations will be made for uninsured children. 

 A basic fee of $55 for physical will be charged at the time of service to those without insurance.
