Water Well Sealing Plan For Homeowners
This form must be submitted prior to a potable water well, boring, or monitoring well being sealed. All abandoned water wells must be sealed in accordance with the Illinois Water Well Construction Code. Approval must be obtained after receipt of this form and prior to sealing such wells. The Livingston County Health Department must be notified 48 hours in advance of any sealing activity.

Water Pump Installation Report
This form must be complete in FULL and submitted within 30 days after a new pump is installed on a well.

Closed Loop Well System Construction Report
Complete and submit geothermal report within 30 days of geothermal well completion.

Sanitarian Consultations for Real Estate Transactions:
- $150 - Well inspection which includes a water sample
- $150 - Septic inspection
- $300 - Both septic and well inspection including water sample
- Three copies of inspection records will be furnished: a copy to each the buyer, seller, and bank
- $5 for each additional copy
- $5 for each faxed copy
Septic Program
- $150 - Septic permit
Water Program
- $100 - Water well permits (state statute)
- $100 - Water modification
- $100 - Surface water use permit
- $0 - Well sealing
- $50 - New well water sample
- $100 - Geothermal Exchange System permit per every 5 vertical or horizontal loop wells
