The Livingston County Health Department’s Community Health Care Program provides two sets of services.  The Public Health Nursing portion of this program focuses on bridging the gap between Home Health agencies and individuals living independently at home who may need a little assistance to make that independent living possible.

The Public Health Nursing program is NOT the same as Home Health.  The program is NOT Medicare-certified.  If an individual might qualify for Home Health, the best action may be to first refer to a Home Health Agency, and then refer to the Public Health Nursing program if Home Health is unable to provide services.

The Public Health Nursing program is able to provide services in the home of senior citizens and disabled clients who need a little assistance to remain living safely in their own home.  There must be a continual need for nursing care. One time only visits do not qualify. These services may include, but are not limited to, the following:

  • Medical Treatments

  • Medication Set-up

  • Blood Draws

  • Home Coumadin finger-stick blood testing

  • Disease Management Education

  • Injections, Immunizations, and other Medication Administration

  • CNA (Nurse Aide) Services

  • Other services as ordered and/or approved by the patient’s primary care physician
