The goal of the Livingston County Community Health Care Program is to assist residents of Livingston County to remain safely in their homes by offering a comprehensive range of services based on an individualized care plan.  The program is made up of two branch programs:  Home Services and Public Health Nursing.

Home Services is a non-medical program that provides homemakers to individuals in need of assistance with everyday tasks and activities, such as personal care, housekeeping and laundry, and meal preparation.

Public Health Nursing provides individuals with medical services delivered by a Public Health Nurse and/or a Certified Nursing Assistant (CNA) to provide care based on a plan developed by the Public Health Nursing Coordinator in coordination with the patient’s primary care physician.  Click [HERE] for more information about this program.
