~ Comprehensive Care Coordination~

This service is available to individuals age 60 and over who are in need of long term care services to remain safely in their home. Comprehensive Care Coordination includes a comprehensive needs assessment, development of a personalized plan of care and care monitoring. There is no fee to receive this service, although some community based services may have fees or eligibility guidelines attached to them. Assessments may be completed on nursing home residents who are considering a return home. Assessments are conducted by highly trained case managers who have special skills and experience in working with older adults.

~ Community Care Program ~

This program is designed for persons age 60 and over who are in need of long term care to remain safely in their home.

Services

 

  • HOMEMAKER - help with housekeeping, laundry, shopping, meal preparation, personal care.

  • ADULT DAY CARE - services are are provided in a supervised setting outside the home.  This is available on a limited basis in Livingston County.

  • Emergency Home Response.

Eligibility

  • 60 or over;

  • Physically or cognitively impaired;

  • A resident of Illinois;

  • A U. S. citizen or legal alien;

  • Assets under $17,500 for a single person - some assets are exempt (special consideration is given to the asset limits for married applicants);

  • Physician approval.

  • Must apply for medicaid.

Cost

Your cost will depend upon level of impairment, type and frequency of service, income level and number of people receiving services.

 

~ Respite Demonstration Project ~

Definition

Respite Care Services to provide relief to caregivers.

Eligibility

Frail persons age 60 and over who receive care from a caregiver.  No income or asset limitations. However, services are targeted to those in greatest economic need with special emphasis on minorities.

  • Institutional Respite

Amount

$500.00 per client per fiscal year (October 1-September 30). Clients may receive additional funds based on availability.
